The intensive recovery programme is a programme which aims to provide care to patients in their own homes or in a community setting bases on their individual needs.
This is a model of intermediate care which offers patients alternative options instead of hospital care.
This programme is closely associated with the process of complex/supported discharge assessments and delayed transfer of care (DTOC) and assessments for CHC and other patient flow pathways.
The main aim of this programme is to manage the closure of 36 inpatient rehab beds and reduce the use of community beds and provide care to patients in their own home with an ambition to implement a single point for transfer of care.
The right candidate must have:
- Project management experience within the NHS
- worked on the same or similar project previously
- worked with community based providers
- worked on a project setting up a new service or new way of operating
- Must be operationally sound and a strategic thinker
